Canonical 404 errors

We noticed after doing a screaming frog crawl that we are seeing tons of 404 errors. When I deactivate Yoast, they are gone. I tried rolling back to a previous version but that did not help either.
For example: https://www.sitemane.com/travel-planner/name/matt-ford/ (correct canonical)
With Yoast active, this becomes the canonical and the 404 created:
https://www.sitename.com/travel-planner/name/michelle-smith/name/matt-ford/All 404’s are doing the same thing with the same name (michelle smith). Here is another one so you can see what I mean.
https://www.sitename.com/travel-planner/name/todd-wilson/ (correct)
becomes this with Yoast activated:
https://www.sitename.com/travel-planner/name/michelle-smith/name/todd-wilson/
We use Connections Business Directory for the travel planners.
