Cannot update record

I am using this shortcode:
[abase form=”1,update” table=my_table_name elements=”facility_id, wssID” columns=”facility_name, facility_id, wssID$Update Facility” ack=”brown” echo=”blue”]
and am getting this error:
Attribute form=””1,update”” is incorrect. Use form=”,search” or form=”,input” or form=”,update”., Table “my_table_name” does not exist
What is being done wrong?
