    Hope you are well!

    This is most likely server configuration problem, and the form you are seeing is a workaround for cases like this.

    WordPress asks for these details when it doesn’t have access to the files and directories on the server.

    This usually happens when the Apache/Nginx server run with one user, and the files and folder are owned by another.

    If you have the FTP credentials from your hosting company, you can enter them in form from the screenshot.
    Or you can just contact your hosting support and tell them that you need to have your files and folders owned by the same user that runs the web server – Apache/Nginx

    Unfortunately, the form for the credentials on the screen shot I provided are greyed out so I cannot enter new details. The only option on the screen is the “proceed” button.

    Since I didn’t create my site, I assumed that the server and hosting were with the same company (Rackspace). I hope they are able to help me over the phone with this issue

    Sorry for the delay.

    It seems I missed the screenshot from your first post, or you added it after I posted my reply.

    If there is already information in this fields and they are grayed out, that means these constants are added to the wp-config.php file in the installation folder.

    You will need to use FTP client, open the file and change the credentials with the working ones. Check your hosting panel, or directly ask the hosting company about them.
