cannot send mails

Strange issue, that the default contact form works fine, but when i created our own form as we need, it fails to send with the red box:
Failed to send your message. Please try later or contact the administrator by another method.https://steinmeyerlaw.com is the site, https://steinmeyerlaw.com/join-a-class-action/ is the form. I found a workaround, but what bugs me is the normal default contact form works fine. So maybe it’s something in the code- but the submit button is the same?
