Cannot place shortcodes for forms

I installed your plugin and I can make the widget work but not the shortcode for a page.
The dialog does not show the accept button and I’m seeing the styles very different from the screenshots you posted.
I downloaded the plugin again to check if there was an installation issue but I keep getting the same errors.
For example I found this too:
SyntaxError: missing ) after argument list
salsapr…ver=0.5 (línea 108, col 9)
ReferenceError: QTags is not defined
admin.p…e=salsa (línea 2773, col 1629)
“NetworkError: 404 Not Found – https://sentencingstg.shoshkey.com/wp-content/plugins/salsa-press/images/salsa-mono.png”
