Cannot make this work

I just cannot make this plugin display anything. Please could somebody help me get this to work? I am trying to use it in the sidebar to display the categories and sub-categories for my pages but all that I see is the widget title. I have tried just using the default settings and changing post type to pages. What am I doing wrong?
