    Unfortunately, the form field will not generate for me. It looks like such an amazing plugin too. With the thought of reinstalling, I did try to clear all references to the plugin from the installation-even went into the dB. Unsuccessful though. Will keep checking back to see if an update fixes my issue.

    The solution for that issue in most cases is to change the script load method to direct, this can be done in the troubleshoot area located below the list of calculated forms in the plugin settings.

    Thanks for the speedy reply! I did change the script method to direct. No change. Prior to activating your plugin, I tred another calculator. I suspect it is causing the issue. When I loaded your plugin into my other website (without the other plugin loaded), the form generated perfectly.
    Will look in forum. Did not see that prior.
