Cannot get form conversions working on GA

Hi,

I am using the universal analytics code (www.google-analytics.com/analytics.js) and have followed the instructions here in setting up:

This is the code that I have added to the additional settings on the Contact 7 form setup page:

on_sent_ok: “ga(‘send’, ‘event’, ‘contact’, ‘submit’, ‘send’, 1);”

In GA I have the following info:

category – contact
action – submit
label – send
value – 1
