Cannot enter address in location field

I have set up the plugin and added the autocomplete field to a test form. However, when I try to enter my address it will only allow me to enter 1 number from my street address. Then it puts an exclamation mark in a gray box over the number. If I try to delete the number, then it closes my page and sends me back to my browser welcome page.

Can you help me figure out what is wrong?
