Cannot Edit Product; Blank Boxes

Hello,

As shown in the imgur link (https://i.imgur.com/AGVFflu.png), when I edit a product in my store, it breaks and goes to a blank page with two boxes. I cannot proceed beyond that, and there is no option in the drop-down menu for the “map” field either.

Please help.
