Cannot delete locations

My website will not allow my to delete locations or upload a location image. There is also an odd location called “Activate %s” that i did not make and cannot be deleted and has no information in it. Should I delete this data from the database? For some reason it is also not letting me create new locations. Any help would be appreciated.
