Cannot Create User Role with All

Hi there,
I have been using your plugin for a while, and I think it’s great!
But today I am having issues creating a new role with all capabilities.
I have tried everything – New roles with all capabilities, giving all roles all capabilities, resetting the user roles. You name it I have tried it… and every time the new user roles gets a very limited amount of capabilities. Any ideas?
Seem to be stuck in a loop of some sort.
