Cannot add form fields

I cannot add form fields to my form because UM is saying the field is already added to my form while it is very clear they are not. The fields appear grayed out and cannot be selected while my form is completely blank. WTF?

Please fix this year old problem already.

I am using Ultimate Member 2.1.13
I have not removed the custom fields and added them back because I don’t know what the exact meta key names are for the fields that need to be added back.
I have multiple custom fields with the same name/label but different meta key names. I can’t tell which one is which.
