Cancel/Remove Attachment & Placeholder text

First, thanks so much for the plugin, it does exactly what I need and I appreciate the work that went into making this.

Second, I might be missing something but is there a way for a commenter to “cancel” or “clear” their attachment? Example, I upload an image and then I decide I don’t want to send that image or any image, I just want to send text. There doesn’t seem to be any way to get rid of the attached file. I even tried refreshing the browser window and it’s still there.

Third, the default WP comment area had placeholder text which seems to have disappeared once I installed the plugin. Is that because the ids changed or something else?
