Can you add a name?

    I have tested this, and it comes back with Phone – and the number, and then Name – and it is blank. Can we have them put their name in there so we know who to phone back?

    Hello.
    The name field is optional for your customers, not required.
    In the request the name will be blank if they not filled this.

    I don’t see a space where they can even fill in their name though… just phone number. Is this with the paid one?

    It (the name field) really needs to be more obvious. Most people wouldn’t even think to click on “you”.

    Can it be made more obvious?

    Hello. Can you offer me how to do this more obvious without increasing complication?

    My goal was to create very simple request form for your customers. More users ask me to move up the name field to the first place. Mostly you need only phone number of your customer. The name is optional. So I will not to complicate a way to getting this.
