can post title be formed from multiple custom fields?

I need to take several fields and merge them into a single field so that when I add it to the Insert and Update Post, it takes the values that were just added and creates 1 single text that becomes the title for example:

Custom text field called Name:
Custom text field called Last Name:
Custom text field named Date:

From another custom field, group the 3 fields into one, in order to create the custom title.
