Can not remove fields from default form

  • I have this plugin on my site, Help me in removing the field. How can I change the default form . I want to remove username from default form. [SS1 > https://ibb.co/Pc95yvT ]

    Also these is a bug with my form when I enter email without password , it takes me to domain.com/wp-login.php but I want there must be a alert for this.

    Help me urgent.
