    In chrome, View > Developer > Javascript console – do you see an error?

    yes, there are the following messages (4 times)

    [jquery.form] terminating; zero elements found by selector
    jquery.form.js:902 [jquery.form] terminating; zero elements found by selector

    Seems it is theme’s issue?

    Did switching theme correct it?
