Can no longer create blogs

I have recently tried to add a few blogs to my multisite install. I currently have 7 blogs on this install (at emergencydental.com). They were all created when I initially built the site in June.
Now, when I try to add a site/blog, it adds it to site list (on the top admin bar and in network admin list of blogs), but I cannot access the site’s dashboard; When I try to go to the dashboard, it always redirects me to the main blog dashboard. It looks like it doesn’t create all the database tables. It just adds the blog name and ID to the wp_blogs table, but it appears to get stuck there. None of the other tables are created.
I initially assumed this was a conflict with one of my plugins, but I disabled all of them, and it still won’t create the blog. Has anyone had similar problems or any insight? Thanks in advance!
