Can I Do This with Contact Form?

I have a basic question and I’m looking for some help

I need to build something that goes like this:

On my page, I will have a product that is typically custom made. I let the customer have the choice of putting their own text on it or we can come up with the text for them. I need a text box where the customer can input their text and I also want a radio button there where they can let us know that they want us to supply the text (and possibly greying out the text box). The customer will then click a button and it will proceed to a PayPal form where they can input their name/shipping info.

I guess essentially I want to have the text field disabled if the radio button is clicked and then have that info forwarded to paypal for payment (PayPal Add-on?)
