calender displaying behind popup form

Hi guys, 2 things I need help with:
(1) the date picked calendar displays behind the popup when you click on the Get a quote button in the first slide. It works fine if the form is on a post or page, but if its placed in a popup , it breaks.
(2) How do I set up a date of birth field, that the starting date displayed is always 18 years from the current date?.
