    Sorry but I don’t see any contact form in the link your provided above.

    There is no setting for this on the form edit page, but you can manually change it.

    Look in this folder on the web server:
    /wp-content/plugins/si-contact-form/date/

    edit this file:
    ctf_epoch_classes.js

    on line 83 change this setting
    from
    this.showWeeks = true; //whether the week numbers will be shown

    to
    this.showWeeks = false; //whether the week numbers will be shown

    You have to make this change again anytime you update the plugin
