Calendar dates form

    I am looking for a way to add a form that will ask for information about upcoming events from people.

    Fields:

    Group
    Contact Person
    Phone
    ——————————————
    Date
    Time
    Activity or Meeting
    2nd date Choice

    The last 4 fields need to be repeated as often as user needs for entering multiple events. I was thinking adding a question or a checkbox if they have more to enter and the form would ask the questions again in some manner.

    To finally email the responses…..

    Possible with this plugin or do I need another plugin? What plugin would you suggest?
