Caldera forms Button not working

I am new to WordPress & NOT a technical person. I created a site (SaxPR.net) and have been learning as I go. The “submit” button does not work on my form. I worked with my hosting provider to make sure everything is set up properly and they say it is. They told me the problem is with the plug-in. It is a simple form. Can someone please help me to understand what I can do to make it work? I googled the problem and any answers that showed up were too complicated for me to understand. It is a simple form and I didn’t do anything complicated with it. I did switch my theme and it did not work with the twenty fourteen theme either, so that doesn’t appear to be the problem.

I did read that it would be good to install a SMPT plug-in, so I did, and the hosting provider said it is configured properly. But still, clicking submit does nothing.
