Bulk Actions not working

The “Select Folder” dialog is no longer displaying.
Also does not work on folders demo site:

In Media Library
In list view
choose some library items by checkbox
choose the “Move to Folder” bulk action
Hit Apply.
The Select Folder dialog does not display.
