Bug: After deleting all default Q/A's the add form disappears

Hello,

After I deleted all default Q/A’s I wasn’t able to add a new question since the add form disappeared after deleting the last remaining question. Instead, only the text “Could not retrieve your settings.” was displayed.

Its easily to fix, just had to move the form code out of the “if” which checks how many rows returned (if ($wpdb->num_rows > 0){).
