Broken email send

My form setup had been working perfectly. I’d sent the final test email having set, tested and re-tested every parameter and it worked. I tested it one last time making no changes in between, and now it has ceased to work!

I have the plugin working on another dev environment running on he same host (one.com), and that continues to work.

Here is the output of the test message (actual, functioning address replaced by [email protected] in code excerpt below). Please help me! My deadline’s strangling me!
