    Hello ivailo333,

    You have to save your form to make the new columns appear. I know that’s a bit counter-intuitive, and improving the UI is in my to-do list.

    Yes, when you have a single column instead of four, as in your screenshots, click the Update button (the big blue button in your screenshots) and the missing columns will appear.
