Booking lost when ‘Success Free Message’ returned

We have reports of customers receiving the ‘Success Free Message’ returned from the Authoirze.net gateway when booking, along with a corresponding ‘Pending’ email. No event or ticket information is included in the email to the Customer or admin, no Booking is recorded, and no transaction is recorded with Authorize.net.
This information is completely lost. We are unable to replicate the issue on demand, unless there is a known way to use the Authorize.net sandbox to trigger the message. Please advise.
