Booking Capacity Exceeded

The capacity of a time slot set to 5.

But it has been exceeded by a user who made two different appointments for the same time slot using two different names (husband/wife), but used the same email address.
Capacity Exceeded, Schedule – Month View
Total number of appointments set at 50 not reached at time bookings were made.
What happened? How do I prevent this?
