Booking form does not work

The booking form should work like this

This theme has the latest plugin version, have you any conflictions with other plugins or the theme?

I’ve been comparing the code of your site and mine, and what I’ve seen is that the js libraries (included in your plugin) are not linked on the html. Should I do it manually?

I have placed the form shortcode on this page:

Not only it does not enter the data in the DB, but it does not send confirmation emails either.

I entered a new booking in the Admin Reservation Area and it registered ok, and also sent the confirmation email.

Can you tell me what is going wrong?
