Please, try to deactivate / activate the plugin.

If this will not help, let me know the version number you are using and URL of page with booking form.

The plugin version is Version 3.8.27

Even under appointments, if I added an appointment manually, it only loads.

Try to do the booking and post here what is in wp-content/debug.log file.

It seems the thread_stack value is low in the configuration of your server. Please, clarify what software are you using to run local server?
