Blank validation message and missing field errors

Hi. I have forms for Login and Registration both experiencing problems with validation and error messages. Here they are side-by-side:

Login outputs a blank validation message at the top and no field errors. Register outputs one of the field errors at the top instead of the validation message and no field errors in the form.

They’re both on the same page but I added them to separate pages with no other forms and got the same results. As a test, I created an entirely new login form but got the same results.

I’ve tried all combinations in Rendering to load with and without AJAX, prevent caching on/off, etc. Nothing seems to make any difference.

My client doesn’t want inline validation enabled so I need to get this validation to work on submit.

There isn’t any caching enabled in WordPress but the host does have minimal caching enabled on their end. In those settings I’ve whitelisted my IP so caching won’t apply to me at all.
