AWeber Integration Not Working

I’ve tried several different things to try to integrate with AWeber. The instructions that I’m trying to follow are at this link: https://premium.wpmudev.org/docs/wpmu-dev-plugins/forminator/. After I sign-in, I don’t get the option to choose a Setup Name and through the rest of the steps. Once signed in, I’m returned back to my Integration page where I receive a confirmation that I’ve been connected to AWeber.
