Avartar images reference non-existant files

I did a fresh install of WordPress 4.6.1 yesterday and am using the default theme Twenty Sixteen. After installing the Avatar Manager plugin and using it to upload a profile image (which I can see in my media library), all avatar image references are broken. This includes the image on the “Your Profile” page, the avatar image next to my blog post, and the avatar image next to my user ID in the upper right hand corner when I’m logged into WordPress.
The SRC attributes for the IMG tags refer to the media library file URL, but inserts the dimensions, such as “-32×32”, into the file name, but those images do not exist.
For example, the profile image file is actually:
https://www.tearfreemonitoring.com/wp-content/uploads/2016/09/daniel_granville.jpgBut the avatar image references insert the dimensions like so:
https://www.tearfreemonitoring.com/wp-content/uploads/2016/09/daniel_granville-32×32.jpgI’ve deactivated the 6 or so other plugins I have installed on this site, but references are still broken.
