auto populate form / filter state / county/ city list plugin

Hello,

Are there any plugins or methods anyone can recommend? I’m looking for a way to have a contact form that allows a user to select a state from a drop down, and then have it populate a second list with only the counties of that state, and then again the list of cities from that county.

I’m sure this can somehow tie in w/ google maps or a database of the US city/state/zip codes.. but I cannot find a single thing online to help me accomplish this (that is within my ability at least)

Anyone able to point me in the right direction for a solution???
