Auto failed test – mistake in LP 4.1.3

Please try t disable each plugin and switch to the default WordPress theme to check.

I try to disable another plugins but it did not help.
I already back to version 4.1.2 which works good, so I can not record the view. But user do not get any message or error. Just after 2 secund it redirect to the last screen of test (with results) with all question skipped and 0% of result…

Also I can see that admin get many emails about finish exam by user – sometimes it 1-2 email, but also it can by 10 this same emails about this same user…

I’m also getting the same issue.
Any advice?

I was able to solve the issue by changing the evaluation setting on course page.
I changed it to Evaluate via quizzes passed.

I am having the same issue. I tried changing the evaluation setting on the course page as per the advice above but the quiz still shows a 0% Fail even though the statistics show that some answers were correct.

The same problem happened with us too. I tried to downgrade 3-4 versions already. Even 4.0.5 and still the same.
Can somebody suggest a fix, please? Is it because we upgrade the DB as well?

will anybody from the dev-team ever reply to this problem? I have been using this plugin for long and advised others as well. I hope I wont regret doing so.
Please help us with this problem asap. Our students are complaining about it.
