Auto-create a "discuss page/post" whenever any new page/post is created

I’m trying to find or write a plugin that would create a “talk” or “discuss” page/post for each corresponding page/post. So, anytime a user creates any new page/post, a discussion page would also be created automatically. This would act very much like MediaWiki’s “talk page”. and I would have a tab at the top to move from “view page” to “discuss”.
