All you need to add to Addition Settings this;
`flamingo_email: “[the-email-field]”
flamingo_name: “[the-name-field]”
flamingo_subject: “[the-subject-field]”

Although, from your first screenshot it’s not clear If you’re even using CF7 correctly.
