Attachments not attached to form data

I created a simple form for artists to submit artwork for review. The form comes through just fine with the info but there aren’t any attachments coming along with the email.

I’ve tried using the default POP as well as SMTP settings, yielding the same results.

Any ideas why the attachment wouldn’t be coming through?
