for me the pictures are getting attached pretty well but when I try to open the images it gives the message as :
    Error 404 – Not Found

Sorry, the page that you are looking for does not exist.
    I have permalinks to : /%monthnum%/%category%/%postname%/
how can I solve this issue. I have set attachments to open up in a page.
if the page is accessed via the attachment id it’s accessible but from direct attach link it’s not opening up ??

when I set attachment to open up via file in attachment it shows up but when set to open up in page it says this 404 error message

Does it work with a different permalink setting?

no haven’t tried as there are more than 500 posts so can’t change it now as it will screw up, my blog links are already in various sites and forums as advertisements and direct links.
I have tried to check out this way:
uploaded file made it as open up in a page, took the code for this and posted in a html page and when I checked it, it opens up well with the thumbnail link to the full image link, but the same link doesn’t open up in blog, giving 404 error message

ATM this is a bug of WordPress, you can use:
    ?attachment_id=(int)
You can use the attribute rel from the form: rel='attachment wp-att-(int)'

    Hope will be useful.
