Attach file or URL based on radio button/drop down

Is there a way that I can get Fast Secure Contact form to attach a file or embed a download URL based on a label field in the extra fields portion of a form?
An alternative would be to do different re-direct URLs based on the label field in one of the extra fields of the form.
I could perhaps use a short code and send the appropriate field query information to the SS Downloads plugin? Not a mysql expert, so I’d need some hand holding..
