Attach a file or form with photos

I’m looking for a way to have a form or file attached to photos or photo albums. I have tried to use the rtmedia attributes add on but i’m not able to attach text. I really would like to have a form attached to a photo or an album where you could click on it and get information on the photo like dates, locations ect. but user added. A form that the user would enter information in would be ideal. Any idea on how this could be done?
