Are you sure you want to do this? (custom admin panel)

  • I have the following form that I got from the source of my custom admin panel for my theme I am designing.

    The issue is, I seem to have everything there but when I test the form and hit submit I get the error Are you sure you want to do this?

    Can some one point me in the right direction how to solve this?
