Appointment form pligin for hospital

This plugin can do that, I have used it in the past as well.

Thanks for your help. But this plugin is paid… Dont we have any free plugin because i am working on a test project. And its $75, $150 etc.
I want a plugin having same functionality.
Please tell me if there are any.
