    Hello, I have four fields on the apply form, all required fields, but only one is showing up on the form when I use the application. Am I doing something wrong? Thank you

    Ive continued to try and figure this issue out. Ive found where if I delete all of the fields from the from in the settings area, then by default ‘name, phone, email, letter, and CV’ fields show up, but if I try to enter my own fields or require any of the file upload fields, only the very last in the list of fields shows up in the apply form.

    Another issue is that when the apply now form is filled out, the email entered is not included with the email notification sent to the admin.

    There was a bug in the plugin which we fixed and updated the plugin on your install.

    Also checked the Custom Apply Form and it worked as expected for us. When adding fields, don’t forget to add Labels to fields. Fields with empty labels will not appear in the form. (We will try to make it more clear in some future release)
