application form with credit card info fields

I’m looking for a plugin that creates a form that has the option of adding fields for credit card info. My client doesn’t want to process credit cards online, just to have the info from this form. It’s an online application for an online school. Is there a plugin that’s good for this?

From my research this isn’t recommended at all, which I told my client. But I just wanted to make sure.

I told them the best way to do it (and least hassle) is to have their customers fill out a pdf and email it to them. Once the email arrives in their inbox, delete it from their inbox & trash immediately. Thoughts?
