App does not show up in the menu

All right. If you have all three plugins:
wpDataTables,
Forminator,
and wpDataTables integration for Forminator Forms you should be able to create tables from Forminator forms.

There is no new settings in admin menu, because options for creating tables from Formiantor forms are in wpDataTables.

What you need to do is to click on wpDataTables in admin manu , then Create table. Checkout image.

After loading page with table wizard you need to choose “Create a table linked to an existing data source” option, and click “Next”. See image

Then choose “Forminator Form” as the Input data source type. See image

After you choose “Forminator Form” as a table type, a new select-box “Choose a Forminator Form” will appear. With this select-box, you can choose a form, quiz or poll, that will provide entries as data for your new table.

After this step you will see a select-box “Choose fields to show as columns” that allows you to choose the form/quiz/poll fields that you will use as columns. Using this select-box, you can choose form fields that will be used in the table.
When you choose what you need, click on Save changes button and table will be created. See image

If you still have issues, please let us know so we will provide you instructions how to send us credentials for your website, because we will need to investigate on your server what is going on. Providing credentials it is not recommended here because of security reasons.
