Any Free Plugin to allow self-maintenance of Contact Details?

Hello All,

I was wondering if there’s a contact-details type plugin available that will allow for people to remotely (from anywhere in the world):

o Enter/add their contact and location related details the first time i.e. if no record already exists in the DB for the person registering
o View and/or edit their (already entered) contact details (when an address/phone number etc. needs to be changed)
o View and delete their contact record (when they no longer want to be part of the contact list)

If possible, the plugin should also have an export functionality, to allow for the export of chosen field(s) to a flat/CSV file…typically in order to allow for the creation of an up-to-date distribution list.
