Any experts of WP Quiz plugins out there?

I’m looking for a WP quiz builder plug-in that can offer the following features…
* it can create Category type personality quizzes with at least 6 categories
* it can create Assessment (score 1-5) type questions, where you rate how much you agree with a statement and that tags a category to same amount (ie Category 1 gets 3 points)
* for some questions it also needs to be able to offer “which answer you agree with” type questions, with each answer tagging one of the same 6 categories
* it can display the top 3 category results at the end
* it can capture email addresses at the end
